CORRELATION BETWEEN HEPCIDIN SERUM LEVEL AND CD4 CELL COUNT IN HUMAN IMMUNODEFICIENCY VIRUS (HIV)-1 PATIENT WITH ARV NAIVE STATUS

HIV infection is a multidimensional problem. Reduction hepcidin expression may limit the replication of HIV-1. Some studies show that changes in iron metabolism can accompany and promote the progression of HIV / AIDS. Serum iron balance is regulated by hepcidin by binding to ferroportin. Down regulated hepcidin expression may limit the replication of HIV-1. Armitagea et al shows that plasma hepsidin significantly higher in HIV patients. The first study of correlation between serum hepcidin levels and CD4 cell count in HIV patients with and without anti retroviral (ARV) therapy reported by Wisaksana et al. This study was designed to prove the correlation between serum hepcidin level and CD4 cell counts in HIV-infected patients with ARV Naive status. An observational study with cross sectional analytic design in the Voluntary Counseling Test (VCT) Clinic and class III ward at Sanglah Hospital. HIV-1 infected patients with clinical stage III & IV WHO aged 15-60 years and never get ARV therapy included. Exclusion criteria: hypoxemia, iron preparations therapy / blood transfusion, malignancy, SLE or RA, chronic kidney disease, chronic liver disease, Mycobacterium tuberculosis infection and pregnancy. Furthermore, the history, physical examination, serum hepsidin examination and CD4 cell count. From 62 samples of HIV patients on stage III and IV, the median levels of serum hepcidin was 41.66 (4.98 to 142.31) pg / mL and the CD4 cell count median was 21 (1-257) cells / m3. In Spearman correlation analysis on these two variables, we found significant correlation between serum hepcidin level and CD4 cell count, with a correlation coefficient -0.28 and significancy value 0.028. From the results of this study, we concluded that there is a negative correlation between serum hepsidin level and CD4 cell count in HIV patients with ARV na´ve status. The results of these studies are expected to be used as the basis for further research in order to prove further the role of inflammation in the progression of disease in HIV.
