ABSTRACT SILICA DUST EXPOSURE IS POSITIVELY CORRELATED WITH SERUM TRANSFORMING GROWTH FACTOR-?1 IN STONE QUARRY WORKERS Silica dust exposure remains a health concern worldwide. Silicosis is a disease that most often occurred as a result of silica dust exposure. Silicosis is characterized by alveolar proteinosis and diffuse fibrosis resulting in progressively restrictive lung function. TGF-▀ is extensively involved in the development of lung fibrosis because of its ability to stimulate the proliferation of fibroblasts and increases the genes transcription that involved in the synthesis of collagen and fibronectin. TGF-?1 is the most abdundant expressed isoform and involved in the development of fibrosis. To date, several studies have not shown consistent results regarding the relationship between silica dust exposure and TGF-?1. This study examined silica dust exposure and serum TGF-?1, which aims to determine the risk of silicosis in stone quarry workers at Company X, Karangasem, Bali. An analytic cross-sectional observational study was conducted at Company X, Karangasem in March 2016. Stone quarry workers at X company, with age 18-60 years, male gender, and have worked at least one year were included, to exclude history of asthma, malignancy, heart disease, hypertension, growth disorder, and connective tissue diseases, and forced expiratory volume first second (FEV1)/forced vital capacity (FVC) <0.7 from spirometry examination. The studied variables were silica dust exposure using nephelometer dust monitoring, TGF-?1 serum using ELISA Quantikine«, with age, smoking, and use of personal protective equipment (PPE) from interview. Spearman correlation test is used to determine the correlation between silica dust exposure with serum TGF-?1. Multivariate linear regression analysis was used to asses the effect of silica dust exposure, age, nutritional status, cigarette smoking, and use of PPE with TGF-?1 serum. There were 55 samples of stone quarry workers in this study. From Spearman correlation test, there was significant correlation between exposure to silica dust with TGF-?1 serum, (r) = 0.319, p = 0.018. In multivariate regression analyzes test, there were significant associations between silica dust exposure and nutritional status with TGF-?1 serum, p = 0.042 and p = 0.012. From the results of this study, it could be concluded that silica dust exposure is correlated with the increasing of serum TGF-?1. Based on serum TGF-?1 examination, it was suggested that stone quarry workers at Company X, Karangasem, Bali have increased risk of silicosis. Recommendations are given to workers, companies, and government to prevent the adverse health effect from silica dust exposure. Keyword : Stone quarry workers, silica dust exposure, TGF-?1.
