Diagnostic Test Of Widal Serology Test Compared to IgM Anti-Salmonella Typhi Test As A Gold Standard to Patient that Suspected of Typhoid Fever at Surya Husadha Hospital in January to December 2013

Typhoid fever is an acute systemic infection caused by gram-negative bacteria Salmonella enterica serotype Typhi (Salmonella typhi), which still has the morbidity and high mortality in the world, especially in developing countries, such as Indonesia. Rapid diagnosis and appropriate methods need to be done as early as possible in patients suspected of having typhoid fever so that patients get proper treatment immediately. Widal test and IgM Anti-Salmonella typhi test is the investigation that often used for the diagnosis of typhoid fever. Based on previous research, it is known that the IgM Anti-Salmonella typhi test had high sensitivity, specificity, PPV and NPV. This study is using a diagnostic test with cross sectional design that conducted at Surya Husadha Hospital in January until December 2013. The purpose of this study is for knowing the sensitivity, specificity, PPV and NPV of the Widal test in comparison with IgM Anti-Salmonella typhi test as a gold standard. In the study showed that of the 203 samples, there are 17 positive samples (8%) and 186 negative samples (92%) with antibodies against Salmonella typhi O antigen, while there are 27 positive samples (13%) and 176 negative samples (87%) with antibodies against antigens of Salmonella typhi H. In IgM Anti-Salmonella typhi test, there are 103 positive samples (51%) and 100 negative samples (49%) with antibodies of Anti-Salmonella typhi. By using the cross tabulation that found sensitivity, specificity, PPV and NPV of Widal test especially for Salmonella typhi O are 14.6%, 98%, 88.2% and 52.7%. While the sensitivity, specificity, PPV and NPV of Widal test especially for Salmonella typhi H are 20%, 94%, 78% and 53%.
